• ☑ I meet any other qualifications prescribed in the Implementing Rules and Regulations (IRR) of RA 9520.

I further declare that I do not possess any of the following disqualifications:

• ☑ I do not hold any elective position in the government (except as a party-list representative being an officer of a cooperative I represent).
• ☑ I do not hold any other position directly involved in the day-to-day operation and management of the cooperative.
• ☑ I do not have a direct or indirect personal interest in the business of the Cooperative.
• ☑ I have not been absent for three (3) consecutive meetings or in more than fifty percent (50%) of all meetings within the twelve (12) month period (unless with a valid excuse approved by the Board of Directors).
• ☑ I am not an official or employee of the Cooperative Development Authority.
• ☑ I have never been convicted in administrative proceedings or civil/criminal suits involving financial and/or property accountability.
• ☑ I have not been disqualified by any other law.
